Technical Analysis

SmileCDR HAPI FHIR does not properly enforce authorization for conditional create operations. When a user with CREATE but not READ permissions submits a conditional create request with a predicate matching an existing resource, the system returns the contents of that resource. This behavior violates intended access controls and leaks resource data to unauthorized users. The vulnerability is identified as CWE-863 (Incorrect Authorization).

Potential Impact

An attacker with CREATE permission but lacking READ permission can gain unauthorized access to resource contents by exploiting the conditional create functionality. This leads to unauthorized disclosure of sensitive data. There is no indication of privilege escalation beyond data exposure or other impacts.

Mitigation Recommendations

No patch or official fix information is provided. Patch status is not yet confirmed — check the vendor advisory for current remediation guidance. Until a fix is available, restrict CREATE permissions carefully and monitor for misuse of conditional create requests.
