The issue involved the chained IRQ handler being set during probe but only removed during driver removal, causing stale data pointers if pci_host_probe() failed. Interrupts were enabled prematurely, and if probe failed, the controller interrupt source was not properly disabled. The fix ensures the chained handler is set only after the INTx domain creation, disables controller interrupts during IRQ teardown, and cleans up IRQ setup on probe failure.
