It arises when group-based policies are evaluated using tokens that contain only group names without full path information. This can cause users in unauthorized groups with the same name as authorized groups to gain access to protected resources improperly. Exploitation requires a specific configuration involving OIDC group mappers emitting name-only claims and group name collisions across different organizational paths.
