It involves a missing cryptographic step in the aes_256_gcm_update module, causing an incorrect GCM authentication tag when using the streaming AES-256-GCM API with empty additional authenticated data (AAD). This flaw results in the hardware GHASH accumulator state not being saved after the first update call, allowing ciphertext modifications without detection by the authentication tag.
