The issue was an off-by-one error in the rollback logic of mlx5_lag_create_single_fdb(), where the cleanup loop incorrectly included the failed index, potentially causing operations on uninitialized state or double rollback of rules. The fix adjusts the rollback to start from the previous index, ensuring only successfully installed entries are undone.
