Specifically, the function nvmet_req_uninit() is called unconditionally even when initialization has failed, causing a reference count underflow. This leads to a warning, a use-after-free diagnostic, and can result in a permanent workqueue deadlock. The issue is addressed by checking the INIT_FAILED flag before calling nvmet_req_uninit(), aligning with existing error handling patterns.
