The function __add_reloc_root() allocates a mapping_node but may fail to free it if a duplicate insert is detected, leading to a potential memory leak. This issue occurs because the error path returns without freeing the newly allocated node when a duplicate entry is found. The vulnerability is a defensive cleanup for an unexpected duplicate insert path, which callers currently assert should not happen.
