The Sophos 2026 State of Ransomware report reveals that 79% of ransomware attacks now begin with compromised identities, overtaking exploited vulnerabilities as the primary entry point. The report highlights the limitations of basic MFA, the accelerating role of AI in threat actor capabilities, and provides detailed mitigation strategies including phishing-resistant MFA, firewall telemetry integration, aggressive exposure management, and immutable backups.

Device code phishing attacks have exploded across the threat landscape, with new toolkits emerging weekly. This surge coincides with publicly released criminal toolkits and multiple phishing-as-a-service offerings like EvilTokens and Tycoon. Threat actors abuse the OAuth 2.0 device authorization grant flow to compromise Microsoft 365 and other enterprise accounts by tricking users into authorizing malicious applications. Current implementations use on-demand code generation, addressing the 15-minute expiration limitation of previous techniques. Most activity appears to be generated using AI-based coding techniques. Successful attacks lead to full account takeover, data theft, business email compromise, and potential ransomware deployment. The technique represents the natural evolution of credential phishing as organizations improve their defenses against traditional multifactor authentication bypass methods.

A large-scale attack infrastructure dubbed TruffleNet has been identified, built around the open-source tool TruffleHog. This infrastructure is used to systematically test compromised credentials and perform reconnaissance across AWS environments. The campaign involves over 800 unique hosts across 57 distinct Class C networks, characterized by consistent configurations and the use of Portainer. Alongside TruffleNet, adversaries are exploiting Amazon Simple Email Service (SES) to facilitate Business Email Compromise (BEC) campaigns. The attackers create email identities using compromised WordPress sites and conduct aggressive cloud reconnaissance. This activity highlights the evolving tactics of threat actors in exploiting cloud infrastructure at scale, combining credential theft, reconnaissance automation, and SES abuse to conduct high-volume fraud with minimal detection.
